Learning from accidents and incidents is imperative for improving all business processes and performance including those for quality, productivity, economic, and health, safety and environment (HS&E). However, all too often, incident investigations do not go far enough to:

  • determine the human factors contribution to an incident (beyond simply attributing it to ‘human error’), and

  • Uncover the underlying organizational weaknesses that allowed that incident to happen.

    Addressing these underlying organizational issues makes it more likely that similar incidents, with similar causes, can be avoided in the future.

    Preventing the incidents is the main goal, however if the incidents do happen determining the root cause of the incident helps companies prevent the same or similar incidents from happening, and also increasing their resilience to hazardous working environments.

Day One:

Root Cause Analysis methodology

  • Process safety management system-pillars,

  • Methodology introduction,

  • Most used Root Cause Analysis methods and techniques,

  • Exercise: Example incident

  • Exercise: Introduction to Software for Incident Investigation and Root Cause Analysis.

    Day Two:

    Accident/Incident Investigation and Root Cause

    Analysis Principles

  • Organization’s defenses between a source of harm and an undesirable outcome,

  • Sequentially Timed Event Plotting (STEP),

  • The four key stages in a Root Cause Analysis,

  • Telling the story through the core diagrams,

  • Exercise: Defining STEP for example incident

  • Exercise: Determining root causes for the example incident.

    Day Three:

    Barriers, Preconditions, Underlying Causes and

    Basic Risk Factors

  • Barrier identification and how they are associated with an incident in a systematic and thorough manner,

  • Human Errors and their link to the Immediate Causes,

  • Preconditions to immediate causes,

  • Underlying causes,

  • Causation path,

  • Basic Risk Factors in Business process

  • Exercise: Creating corrective actions based on the example incident root cause analysis.

    Day Four:

    Remedial Actions, Reporting and Corrective Actions Plan

  • Identify where remedial action should be applied,

  • Haddon‘s hierarchy of barrier effectiveness,

  • The cost vs. benefit of various types of barriers,

  • Report layout and format,

  • The ways in which lessons learnt may be communicated throughout the Company,

  • Exercise: Creating a final report
